i've seen a tremendous amount of cd packaging & the one that comes to mind is Tesla's Reel to Reel - having the CDs as master tape reels was kinda cool. good design.
i always liked KMFDM's artwork ... i think the majority of it was by Brute.

My favorite album artwork is always TOOL from Opiate on, thier album art is the best!
I like A Perfect Circle's Album art as well.
Hurt's Vol. 1 and 2 are sweet!
I love the cover art of Pearl Jam's VS.
